delivery of stock shares is the most secure and expedient delivery
process available and provides efficient internal control as well
as cost savings. However, you may also transfer certificates
directly. To help you facilitate a gift of stock, please use
the following instructions:
Notification
of Donor
Intent
Regardless of the method chosen to deliver a
gift of stock, the donor or the transferring broker must provide
the following information for audit and acknowledgement purposes:
Donor's name, complete address and
telephone number
Name and number of securities transferred
Parish, school or fund to which the gift
is directed
You may use the gift form, call,
fax or e-mail to communicate this information at the time of

Stock
certificate:
If the stock certificate has been re-registered
to The Roman Catholic Bishop of Monterey or to the Diocese of
Monterey, it may be submitted without additional paperwork from
the donor.
If the stock certificate is in the name of
the donors, the owner(s) need to go before a Notary Public and
sign the back of the certificate exactly as the stock is
registered, and make no other notations on the form. At the
same time, the owner(s) must sign and notarize a Third Party
ReleaseForm to the above address in Monterey.
